1. Data controller

This website is not for booking appointments or sending health information. Appointments are booked through znanylekarz.pl, which is a separate data controller with its own privacy policy.

Data from the form is processed on the basis of Article 6(1)(a) GDPR (consent) in order to answer your question, and Article 6(1)(f) GDPR (legitimate interest) in order to handle correspondence.

4. How long we keep data

We keep correspondence for as long as needed to answer and to deal with any follow-up questions, and no longer than [TO BE COMPLETED: e.g. 12 months] after the last contact.

5. Recipients of data

  • Cloudflare, Inc. — website hosting and visit statistics.
  • Web3Forms — delivery of the contact form (when the form is enabled).
  • The e-mail provider that receives the correspondence.

6. Cookies

This website uses no marketing or analytics cookies. Visit statistics are collected anonymously, which is why there is no cookie consent banner. Fonts are hosted on our own server — your browser does not connect to third-party servers to load them.

Playing a video embedded from YouTube or Facebook does connect to those companies’ servers and may set cookies — which is why such material only loads after you click it.

7. Your rights

You have the right to access your data, to rectify it, to erase it, to restrict processing, to data portability, to object, and to withdraw consent at any time. You also have the right to lodge a complaint with the President of the Personal Data Protection Office, ul. Stawki 2, 00-193 Warsaw, Poland.

8. Physiotherapy records

Data collected during a visit to the practice (history, examination, treatment records) is processed under separate rules arising from medical records legislation and is not covered by this policy. You receive information about that processing at your first visit.
